2007 Final Outcome Reports

School Challenge grant final outcome reports are due one year after the grant is awarded
Schools that receive a Highmark Healthy High 5 School Challenge grant from the Highmark Foundation in 2008 are required to submit a final outcome report no later than one year from the date the grant was awarded. Final Reports can be sent to the Highmark Foundation at finalreports@highmarkhealthyhigh5.org or through the U.S. mail. Note: If you are sending your report via e-mail, please include the school district and school name in the subject line of the e-mail.

Your outcome report should include answers to these three questions.

  1. Briefly summarize your project by telling us what you did and what you hoped to accomplish (report outcomes in question #2). What is the number of children who participated?
  2. Report your outcomes: What were your most important measures of success, and how did they change over time? Report the average values at the beginning of your program, and the average values at the end of your program, and the number of children measured at each time point. Are these findings what you expected?
  3. Do you feel your program was successful? What worked and what did not work? If your program did not work out as planned, what would you do differently next time? What advice do you have for others implementing similar programs?

Limit the report to one to two pages, plus any attachments.

Outcome measures should be based on the measures you listed in your proposal. Please report your major outcomes by providing the average score or measurement at the beginning of the program and the average at the end of the program for each of your outcome measures. Include the number of children measured at each time point. If you wish to provide additional supporting data, please include them in an attachment.

  • If your outcome was to show improved fitness, strength or endurance, provide average fitness test scores at the beginning and end of your program.
  • If your outcomes were based on fitness or nutrition logs kept by the children, select at least one common item for the logs (# repetitions, time spent in an activity, number of activities, number servings vegetables, etc.) and have the children report on this value at baseline and at the end of the program. Average these values at both time points and report this as your outcome measure.
  • If your measurement was to increase the number of children participating in an activity, report average participation rates before implementing and at the end of your program.
