All applicants for Highmark Healthy High 5 funding should complete our grant eligibility screening before completing a grant application.

The grant screening will help you determine if you are eligible to apply for a Highmark Healthy High 5 grant for nonprofits or a Highmark School Challenge grant. Eligibility to apply for a grant does not guarantee a grant request will be approved.
