The first step of the Highmark Healthy High 5 grant application process for 501(c)3 nonprofit organizations is to complete the eligibility screening. Eligible nonprofits must then submit to the Highmark Foundation a two- to three-page letter of inquiry. A letter of inquiry must be submitted on organizational or program letterhead, and must be signed by the administrative head of the applicant organization.

Upon receipt and review of a letter of inquiry, the Highmark Foundation will notify an applicant about the status of a request. If a request meets all criteria, Highmark Foundation will invite the applicant to submit a formal proposal. Learn more about the Highmark Foundation.
