Final Outcomes Report
Highmark Healthy High 5 School Challenge Grants
To improve the health and well-being of our children
Schools awarded Highmark Healthy High 5 School Challenge grants are required to submit a final outcome report as per the date outlined in your grant award letter.
Please complete a 2-3 page report that includes responses for each of the following questions. Please email your report to finalreports@highmarkhealthyhigh5.org. Put your school’s name in the subject line of the email. Or send by US mail to: The Highmark Foundation, 120 Fifth Ave, Suite 2585, Pittsburgh, PA 15222.

- Name of school
- Name of person completing report
- Address
- Phone number
- Email address
Please answer the following questions in your 2- to 3-page narrative. If you had any news stories published about your program, please include as an attachment.
- Briefly describe the program you implemented. Did you develop the program or is it a national or regional program? What did Highmark Healthy High 5 funds pay for? If other funders contributed to the program, explain what aspects of the program were funded by the Highmark Healthy High 5.
- What was the purpose of your grant? What were the goals and objectives? If these changed after you received funding, please explain.
- How many children participated? How many times per week and for how many minutes each time? Approximately when did your program start and end? If it is still going on, when did you collected the follow up data? If parents were involved, describe how.
- How did you measure your success in achieving goals and objectives? Summarize the results of any quantitative data collected. In most cases, you will report the average value before and after your program, and the number of children measured at both time points. Please include as much data as you need to explain your outcomes. You may include attachments, but please do not include children’s names.
- Were you successful, in full or in part, in achieving your objectives? Please explain. Include any other indications of program success, including your personal perceptions.
- If you were not as successful as you hoped, what did you learn from the experience?
- What advice would you have for others who want to implement a similar program?
- Will your program continue next year? Why or why not?
- Do you think the program had or will have any lasting impact on the school, the children, policy, or on how you do things? Please explain.
- Based on your experience with this program, what would you like to try next?
- Did you promote the Highmark Healthy High 5 grant or program through the template press release that you received, in school announcements, bulletin boards, etc? If yes, please identify how you promoted Highmark Healthy High 5.
